What You’ll Do

As our Social Worker, you’ll serve as a trusted guide for residents and families—helping them navigate the emotional, medical, and practical challenges of long-term care.

Your key responsibilities will include:

  • General Functions
    • Ability to move freely through campus to interact with clients, residents, providers and staff
    • Ability to recognize problems and recommend solutions
    • Knowledge of unique problems and abilities of nursing home residents and their families
    • Knowledge of federal, state and local codes and regulations
    • Ability and willingness to function as a member of an interdisciplinary team 
  • Pre-Admission & Admissions
    • Conduct tours and interviews with potential residents and families
    • Explain admission agreements, rights, and resident handbooks clearly and compassionately
    • Ensure all required documentation is signed and processed
    • Welcome and orient new clients / residents to their room, staff, and daily routine
  • Ongoing Support & Advocacy
    • Build relationships with clients and families to understand their needs and preferences
    • Serve as an advocate for residents’ rights, including advance directives and care preferences
    • Provide emotional support and education around insurance, Medicaid, Medicare, and care transitions
  • Care Planning & Coordination
    • Develop comprehensive Social Service Care Plans, Social Histories, and Discharge Plans
    • Participate in interdisciplinary care conferences and discharge planning meetings
    • Complete MDS assessments accurately and on time
  • Discharge & Transition Planning
    • Coordinate discharges and transfers with receiving facilities and community agencies
    • Arrange supportive services and durable medical equipment as needed
    • Provide timely communication to families and follow up to ensure smooth transitions
  • Regulatory & Community Responsibilities
    • Maintain compliance with state and federal codes
    • Report discharges to the Ombudsman and Social Security office
    • Participate in QAPI (Quality Assurance & Process Improvement) efforts
    • Lead staff education on resident rights and social work best practices

What You Bring

  • New graduates of Social Services programs may be considered
  • Bachelor’s degree in Social Work or related field (social work preferred)
  • At least 1 year of experience in a healthcare or long-term care setting
  • Knowledge of Medicare/Medicaid, insurance plans, and state/federal regulations
  • Empathy and patience working with elderly populations and families during life transitions
  • Strong organizational, communication, and documentation skills
  • Ability to collaborate with medical staff, caregivers, and community partners
  • Computer proficiency for charting and reporting
